English to Russian Medical Medical: Pharmaceuticals

For each patient an experimentation form was prepared with no. of diseases of the respiratory organs occurred in the same quarter of the previous year, the diseases occurred during treatment period, gravity of the disease at the start and at the end of therapy (surveys were made also 1 month and 2 months after start of treatment), initial and final presence of symptomatologies such as rhinorrhea, headache, dysphonia, pharingeal hyperemia, cough, sputum and dyspnea and initial and final auscultation and percussion reports.
Symptoms were classified as follows: 0 = absent ; 1 - slight ; 2 = present ; 3 = accentuated ; auscultation and percussion reports were classified as follows : 0 = normal, 1 = slightly alterated, 2 - alterated, 3 = seriously alterated.
