"l sottoscritto XX XX, nato a (X ) on xx/xx/xxxx, acquisite le informazioni fornite dal titolare del trattamento al sensi dell’art.13 del D.lgs. No.196/2003 su modulo aziendale DPS007, e consapevole, in particolare, che il trattamento riguardera I dati “sensibili” di cui all’art.4 comma1 lett. d), nonche art.26 del D.lgs 196/2003, vale a dire I dati 'idonei a rivelare l’origine razziale ed etnica, le convinzioni religiose,...' ...." etc

I am confused about who acquired information from who. Is it enough just to say "xxx...., "having been informed by the data processor"? Or would that mistranslate the idea?

I think it would mistranslate... "fornite da ... su ..."
The information about data protection is contained in a separate document (DPS007)

that the patient is given to read before signing the consent form.
